Position Overview:
The primary responsibility of the Community Health Worker is to serve as an on-site HealthySteps specialist, providing care coordination and connecting families to critical resources.
Reports to: Director of East Tennessee Programs
Primary Responsibilities:
Complete training for HealthySteps implementation and maintain fidelity to the program
Provide universal screening using tools like PSC-17, SWYC, and ASQ-SEs
Stratify children and families to appropriate tiered interventions based on screening results
Connect families to Social Determinants of Health resources and tele-behavioral health services as needed
Collaborate with Identified Practices and integrate services into well-child and office visits
Support the implementation of Help Me Grow as a backbone contact

Minimum Requirements:
High School diploma required (Bachelor’s degree preferred)
Proficiency in Microsoft Office (Word, Outlook, Excel) and Google Drive
Experience in a medical and/or clinical setting preferred
Required Skills and Abilities:
Excellent oral and written communication skills, demonstrating professionalism in all forms of communication
Strong organizational skills and self-motivation
Ability to work effectively within an interdisciplinary team
Capacity to collect, analyze, and report data, and engage in feedback loops
Location: Newport, TN
Compensation: $35,000 - $40,000 per year
